My Personal Head & Neck Cancer Daily Treatment Note Template
Made available January 2025
After receiving multiple requests, I’ve decided to share my personal documentation templates! This is the same daily note template I use in an outpatient hospital setting with EPIC EMR, though it can be easily adapted for other EMR platforms.
If your EMR does not allow custom smart phrases, you can still make use of the template by saving it as a Word or Google document. Simply copy and paste the relevant sections into your EMR to streamline documentation and maintain consistency.
I’m in a general outpatient clinic (not head and neck specialized) and primarily see patients who are post-surgery undergoing chemoradiation/radiation treatment. More complex head and neck cancer patients are typically not treated at my hospital. As this is my personal template, it reflects my patients and would be best for SLPs seeing CRT/XRT patients.
What’s Included:
The head and neck cancer daily note template in both Word and PDF format
A Key document (also in Word and PDF) with example wording for each placeholder in the template
You’ll need Microsoft Word to edit the files directly. The PDF allows you to copy and paste into your EMR.
